1.  Complete the online registration form

• It is REQUIRED that each team submit this form.
• Each SCHOOL that the athletes attend should submit registration and payment.

2.  Submit full team roster with this form

• You will also upload your FULL team roster at the end of the form.  Please have your FULL roster formatted as a spreadsheet available. 
• The roster should include the first and last name, gender, and grade level (5,6,7,8,9,10,11,12) of each swimmer. 
• If you are a public school with Homeschool and/or Virtual School athletes, you must indicate the homeschool/virtual school students on the roster.
• Your submission of this form means that you are certifying these swimmers attend and are registered with your school. If questioned, you should be able to present reasonable verification of enrollment.  You must submit the roster to be fully registered.  You may send updates to your roster and registration until the state meet entry deadline.

3.  Submit payment to MTHSSA

• Registration dues are: 
• Before December 1: $2 per swimmer. 
• From December 1-January 5: $2 per swimmer + $25 per team. 
• If received after January 5, teams cannot register for the Region Championship meet.  
• Please use the payment form.   It is very important to use the payment form so that the correct credit can be applied.  Please note the PO Box change.

If applicable----Middle School Co-op Supplement (required for all teams wishing to co-op for the MTHSSA Middle School Region meet)  Rules pertaining to coop can be found in the bylaws.

*If your team has missed time fees from the Regional meet then your team's registration will not be considered complete until the fees are paid.
